FOUR LEVELS OF CARE YOU ARE ENTITLED TO RECEIVE
Routine Care: Empire Hospice Care provides residential care and has established partnerships with long-term care facilities and nursing homes, assisted-living facilities and residential care facilities for the elderly.
Continuous Home Care (Intensive Comfort Care): Empire Hospice Care provides multiple shifts of hospice care from our licensed nurses and hospice aides for up to 24 hours a day in order to help the patient remain at home through the end-of-life transition or for management of acute medical symptoms in order to achieve palliation
General Inpatient Care: Empire Hospice Care has special arrangements with local nursing homes and other supporting facilities where patients may receive the necessary and appropriate inpatient care in order to address any additional needs beyond what can be safely managed at home. General Inpatient Care provides pain control and symptom management that cannot effectively be provided in other settings.
Respite Care is short-term inpatient care limited to five consecutive days provided to the patient when necessary for the purpose of providing a break in caregiving to the patient’s caregiver(s). It is only provided on an occasional basis approximately every 90 days. Respite Care is provided in a hospital, hospice unit or long-term care facility without meeting the “inpatient” pain and symptom management criteria.
